Have you read the relevant documentation page about the Form Builder?

It's a bit of a boring read, but it does contain all the info to get you going I think!

It's possible that you're experiencing a bug which prevents it working like it should, in which case I'd need a bit more information to track it down. What exactly doesn't appear to happen, which you would expect to happen?

Thanks for the link to the documentation page. I think I can work out how to set the questions now. Ideally I would like to add in "information" in the from of text between sections or questions. Is this possible?

You can't add information between questions - the best you can do is to use appropriate section headers, as well as setting suitable help text on each question.

Another option (depending on your form requirements) would be to use the Survey tool, which allows more complicated sections with their own introductory text, as well as having fancy features like showing different form sections based on previous answers.
