The Trust undertakes safety huddles after each seclusion, discussion takes place in the Reducing Restrictive Interventions Group and through training under the Bild Association of Certified Training Standards.

•      Increased dose of antipsychotic

How does the Trust plan to prevent medication errors in the future?
RDaSH will implement our Patient Safety Incident Response Framework (PSIRF) policy to reduce the chances of error.
