Some of this information is not recorded in a format that would make it possible to extract within the notional, statutory time limits of the Freedom of Information Act 2000, as stated in Section 12 (18 hours/£450). The Trust confirms that it does hold this data, however to extract and collate the data would require one officer of the Trust manually accessing individual patient files for all patients to find the information relating to the questions asked. At present, the trust has provided as much information as possible within the timeframe, and to extract anymore information would exceed the 18 hours/£450.

Family member concerned that they had been prescribed medication that they may have previously had a reaction to- investigation found that no doses had been administered and prescription was changed once staff became aware of potential risk.
