OUR RESPONSE: The Trust has a least restrictive practice plan that is reviewed yearly aimed at supporting the reduction in the use of restraint and seclusion. The Trust train clinical staff in preventing the use of restraint and seclusion by alternative methods utilising Safewards and Positive Behaviour Support approaches and if restraint is required it is done by trained staff using Safety Interventions training. All restraint and seclusion incidents are monitored monthly in services and at the Trust Least Restrictive Practice Group.

OUR RESPONSE: I can confirm the Trust holds the information you have requested. However, we are withholding this information since we consider the exemption under Section 40(2) of the Freedom of Information Act 2000 applies to it, as it constitutes the personal data of the individuals concerned. Although investigations do not all contain names of individuals; patients and staff could be identified by the description of the particular incidents and the outcome.

OUR RESPONSE: 197 occurrences.

OUR RESPONSE: 148 occurrences for men and 49 occurrences for women.
