Thank you for your request for information under the Freedom of Information Act 2000, which we received on 26th April 2021 regarding ECT, serious incidents, restraints, seclusion and medication errors.

  5. What proportion of patients were men/women?

  7. What proportion of patients were classified BAME?
  8. How many were receiving ECT for the first time?
  9. How many patients consented to ECT?
  10. How many ECT complaints were investigated outside the NHS and CCG?
  11. How many patients died during or 1 month after ECT and what was the cause (whether or not ECT was considered the cause)?

OUR RESPONSE:  

From our preliminary assessment, we estimate that compliance with your request would exceed the appropriate costs limit under section 12 of the Freedom of information Act 2000, currently £450, as much of the information is not centrally recorded and we would need to check multiple sets of records.
However, in an attempt to be helpful, we are able to provide the following information.

ECT Information:  
 
  1. Please see attached leaflets.
  2. Please see attached ECT consent form.
  3. Patient has full anaesthetic work-up and has to meet the NICE guidance on use of ECT. Any investigations will be done if required.

  5. 53 Women and 21 Men.
  6. Age range of 22 to 87 years old.

  14. One patient suffered an asystole event and had further investigations prior to any further ECT to ensure it was safe to continue (which it was).
  15. Yes one complaint.
  16. One patient needed to be treated in theatres and our Acute Hospital Trust were unable to offer a theatre slot in a timely manner, patient was waiting for treatment. The family were not happy.
  17. No patients have had a referral to a specialist memory/cognitive assessment post completionof ECT for memory loss or cognition loss.
  18. Montreal Cognitive Assessment Tool (MOCA) and also 5 orientation questions pre- treatment and post treatment.
  19. No
  20. ECT given as a last resort, urgent treatment option so no plans to prevent future ECT.However other options are being considered such as RTMS and VNS.

Medication Errors 

We can inform you that 772 medication errors were recorded for 2020.
