Medication Errors

 

OUR RESPONSE: From our preliminary assessment, we estimate that compliance with your request would exceed the appropriate costs limit under section 12 of the Freedom of information Act 2000, currently £450, as much of the information is not centrally recorded and we would need to check multiple sets of records. However, in an attempt to be helpful, we are able to provide the following information.

  3. Patient has full anaesthetic work-up and has to meet the NICE guidance on use of ECT. Any investigations will be done if required.

  7. 1 patient experienced asystole reaction which self-rectified. They were transferred to A&E as a precaution.

  9. Patient’s relatives complained as patient had to wait to have treatment in main theatres due to significant underlying condition. Treatment was given following delay.

Medication Errors

We can inform you that 743 medication errors were recorded for 2019. Please find attached a copy of our Medication Errors policy.
