18. What tests are used to assess memory loss/loss of cognitive function?  
 
The Montreal Cognitive assessment is conducted prior to first treatment, after every 4 treatments, 4 days after the final treatment and at the 4, 8 and 12 week follow up. We also use the CPRS on memory at every treatment.
 
19. Have MRI or CT scans been used before and after ECT?  
 
MRI and CT scans are only requested as would be clinically indicated they are required.
 
20. If so what was the conclusion?  
 
The ECT clinic does not hold this information
 
21. How does the Trust plan to prevent ECT in the future?  
 
People are referred to the ECT Service as clinically indicated by NICE and the Royal College of Psychiatrists. It is an effective intervention for many people as currently we plan to continue to use ECT as clinically indicted to do so.

We have a comprehensive Medicines without harm Quality Improvement programme in place - this is actively managed and monitored through our executive led, multi professional, Medicines Safety Committee (MSC) –a sub group of our Quality Assurance Governance Committee (QGAC) and the Operations Board which report directly to the Board. Key drivers of this programme include; in-house clinical pharmacy team, strengthened, proactive sharing and learning from incidents through medicines incident groups, regular medicines safety briefings, exploring introduction of trigger tools within Medicines Reconciliation (MR) processes for critical / high risk medicines (at all transitions of care including discharge), comprehensive Medicines Optimisation scenario based training programme, procurement and implementation of electronic prescribing and medicines administration system, linked to SMART cabinet functions, improving patient and service user involvement through shared decision making, regular medication review and enhancing access to and use of all stages of self-medication.
