13. How does the Trust plan to prevent MEDICATION ERRORS in the future?
Response: All Medication incidents are reported on the trust incident reporting system which are viewed by the medicines management group. The medicines management group actively monitors and analyses all medicine incident reports, to identify themes across the Trust which require system reviews or change in processes to prevent or minimize medication errors and promote cross learning of good practice. Medicines Management training is provided to staff and continually updated to reflect new guidance.
