Note 1:
The Freedom of Information Act 2000 (FOIA) does not require public authorities to create data in response to a request. To provide this information the Trust would be required to review the individual medical incident record to review the individual’s medical record and if required, contact other Trusts to verify information regarding ECT, Serious Incidents, Restraints, Seclusion or Medication Errors.
