I am writing to confirm that the Somerset NHS Foundation Trust has now been able to consider the your appeal of Section 12 of the Freedom of Information Act 2000.

We have provided information where it is feasible within the timescales. However, we still uphold that providing this full information would be in excess of 18 hours.

Taunton and Somerset NHS Foundation Trust and Somerset Partnership NHS Foundation Trust merged on 1^st April 2020 to form Somerset NHS Foundation Trust.

Under Section 12 of the Freedom of information Act does not oblige a public authority to comply with a request for information if the authority estimates that the cost of complying with the request would exceed the appropriate limit.

We estimate that the cost of complying with your request would exceed the appropriate limit of £450. The appropriate limit has been specified in regulations and for NHS Trusts it is set at £450. 5 reported mild short term memory loss

What tests are used to assess memory loss/loss of cognitive function?

Standardised tests now being implemented following ECTAS accreditation, however in 2020 standardised tools were not routinely carried out. This was done on an individual basis. The clinic now requests a memory and cognitive assessment is carried out prior to commencing ECT and throughout the course.

Have MRI or CT scans been used before and after ECT?

If it is clinically indicated the referring medical team will arrange this, but this is not a standard requirement.

How does the Trust plan to prevent ECT in the future?

ECT remains an invaluable and safe treatment for those patients who are clinically indicated and is only administered after thorough assessment. The trust are developing other Neuromodulation treatments to offer some patients alternative treatments.

Patient felt they were badly mistreated, restrained against their own will and made to do things against their own wishes. They wanted an apology and to understand the procedure and protocols that were in place.

	Description
1	Patient believed he was given wrong medication which resulted in them getting diabetes. This was not upheld.
1	Failure to give patient their medication with they felt led to a cardiac event?
1	Wanted to know why medication was stopped and nothing given in there place?
1	Patient was discharged with wrong patients medication.
1	Changed to medication cause patient’s heart block or lowering of blood pressure?
1	Patient accidentally given Oramorph when allergic to it.
