  23. How does the Trust plan to prevent ECT in the future? - The Trust is committed to using evidence-based treatment as prescribed by NICE. Transcranial Magnetic Stimulation is NICE indicated treatment that is being considered as an alternative to ECT where indicated.

  8. If so, what was their concerns? - The descriptors of these concerns were:

  • Wrong dosage of medication given

  • Near miss – medication administration

  • Problems with medication

 

  1. How does the Trust plan to prevent MEDICATION ERRORS in the future? - The Trust prepares quarterly medication incident reports containing an analysis of themes.  These reports are tabled at the Medication Safety Group, Drugs and Therapeutics Group, Network Governance Groups and are also sent to Commissioners.  Medicines management nurses review all incident reports involving medication and provide immediate support e.g. additional training, competency review where this has been identified as a need.  The Medication Safety Group agree any additional action needed in response to key themes from the quarterly medication incident reports and incorporate these into the Medication Safety annual work plan.
