4.How many ECT in 2023? 179 instances (21 patients)

9.How many people were offered talking therapy prior to ECT ? This is information was not previously captured, we have now updated our paperwork to ask the question but we are also asking if patients have had any psychological interventions.
10.How many were receiving ECT for the first time? 11 patients In 2023 for both Sandwell and Wolverhampton, Dudley and Walsall.(4 localities)
11.How many patients consented to ECT? 13 patients consented to ECT for the 4 localities
12.How many ECT complaints were investigated outside the NHS and CCG? We have not had any complaints.
13.How many patients died during or 1 month after ECT and what was the cause (whether or not ECT was considered the cause)? None

14.How does the Trust plan to prevent MEDICATION ERRORS in the future? Each medication error reported is investigated locally and if required this can be escalated through management for additional oversight. Individual learning needs / actions are taken where appropriate. Where patterns are found, additional spot teaching / process review can take place up to and including Trust wide policy review. EPMA is being brought into the Trust in 2025 to automate some medicines processes and to reduce some forms of human error and build in new safeguards. Training of clinical staff is a central part of the reduction of medication errors. We are currently reviewing the medication error policy and competency framework for clinical staff covering , prescribing, administration and dispensing/ supply errors. Medication errors and patterns/concerns are shared Trust wide at medicines management committee and Quality & Safety Steering Group as well as at Quality & Safety Committee (Trust wide) on a quarterly basis.

*<5 - Section 40 (2) Personal Data exemption applied - Due to small numbers involved within these responses we have indicated a value ‘less than’ 5 this is to ensure that individuals are not identified as other information that could be within the public domain may aid the identification of the individuals

*Section 12 (1) Cost/Time Exemption applied – The request is estimated to take longer than the provision of 18.5hours.
There are 1300 deaths case notes to review.
1300 deaths x 10minutes to review = 13000 = 216.6 hours.
